I wish there was a tense in English that separated perfect modals, because there are different meanings. If i say: Mike could have gone home. This is saying perhaps Mike went home, the outcome is unknown at present. But it I say: Mike could have gone home, but he chose to stay at work longer. This is saying Mike could have chosen to go home, however, he decided to stay at work instead. Logically, there should be a tense that describes possibilities that could have existed in the past but were not actualized. And it should be different from possibilities that could still be true today. And yet both of them use the same structure in the beginning. In my brain, for instance if I say something like : Mike could have had gone home, but he chose to stay a while longer. This is saying he had the chance to go home, but he decided not to. Which is exactly the meaning I was trying to imply, but it is grammatically incorrect. You can use modal + have + had But only if there is a noun like: Sarah could have had her hair cut yesterday, but she got sick. In this case it’s considered correct because someone else is doing the action.
