Treatment of Henoch Schonlein Purpura: - Usually Supportive. - Indications for Steroid: 1) GIT manifestations. 2) Neurological Manifestations. 3)Renal Manifestations. 4) Testicular swelling. P.S: Steroids is not recommended for skin rash nor for arthritis. #pediatrics

In Megaloblastic anemia: Neurological manifestations point towards Vit. B12 deficiency. while: GIT manifestations ( abdominal pain, chronic diarrhea) point mostly point towards Folate deficiency. #Medicine #pediatrics

RDW: The RDW measures the variation in RBC size. The RDW is usually abnormal increased in iron deficiency, but is generally normal in all of the other microcytic anemias. #Medicine

Causes of Death in thalassemia: 1- Heart failure may be due to: - Under transfusion ~ anemic Heart failure. - Over transfusion ~ volume overload. - Cardiomyopathy due to hemosiderosis. 2- Hemosiderosis: - Diabetes mellitus ~ Diabetic ketoacidosis. - Liver cirrhosis ~ Liver cell failure. 3- Hazards of frequent blood transfusion: - Allergic reactions. - Pyrexial reactions. - Incompatible blood transfusion. -Disease transmission (HBV, HCV, AIDS) 4- Hypersplenism ~ pancytopenia 5- Splenectomy ~ Sepsis. #Medicine #pediatrics

ESR: - طبعا هو تحليل شائع ويستخدموه اغلب الدكاترة بمختلف الاختصاصات بس قيمته حديثاً دتقل كونه non sensitive and non specific. - it is the ability of RBC to sediment. - حالياً يستخدمون automated analysis حتى يعرفون قيمته بس نكدر نسويه مختبرياً بأن نجيب test tube بيه anticoagulants ونسحب دم من المريض بيه ، ونراقبه راح يبدي يترسب ثلاث طبقات packed RBC puffy coat plasma بعد ساعة راح نقيس بالمسطرة الطول مال packed RBC وحدته طبعاً mm/hour - بما انه هو rate ف يمثل التغير بالمسافة مقسوماً على التغير بالزمن لذلك لمن يمر وقت sedimentation increases but ESR remains the same. - normal ranges: اكو هواي مصادر بس مرات للتسهيل يستخدمون هذي المعادلة ESR(mm/hr)= age(in years)+10 ( if female)/2 - الاشياء اللي يعتمد عليها هي - rouleaux formations - Immunoglobulin level - negative charge اول اثنين يتناسب وياها طردياً واخر واحد عكسياً - طبعاً يبدي يصعد 24-48 hour after the onset peaks at 3-7 days return to normal after weeks. لذلك يفضلون CRP بال follow up لان يرجع للنورمال اسرع.
