X ray of croup patient the steeple sign is a radiologic sign found on a frontal neck radiograph where subglottic tracheal narrowing produces the shape of a church steeple within the trachea itself The presence of the steeple sign supports a diagnosis of croup, usually caused by paramyxoviruses.

Croup, also known as laryngotracheobronchitis. بالعربي يسموه الخناق او الخنيزيره is a type of respiratory infection that is usually caused by a virus. The infection leads to swelling inside the trachea, which interferes with normal breathing and produces the classic symptoms of "barking/brassy" cough, inspiratory stridor and a hoarse voice. Fever and runny nose may also be present. These symptoms may be mild, moderate, or severe. So we do the sample test to the patient - We inject a small amount of antibiotic under the skin and wait for one to five minutes. -If the patient show the signs of allergy (Itchy, watery eyes,Rashes,difficulty breathing) Stop this antibiotic and switch to another one.

Very interested case( Congenital heart disease) 19 days old boy presented to the Emergency department with shortness of breath O2 saturation 70% in room air. Cardiac auscultation show holosystolic murmur. Respiratory auscultation normal breathing. Send the patient for full investigations , especially Echo. The result of echo show * Tricuspid atresia with large ASD and large VSD. Admitted the patient to the ward for complete management and investigation.

** Spot diagnosis - Mercury poisoning (تسمم الزئبق)

3y old girl presented to the ER with foreign body swallowing ( witness ). After auscultation the chest ( good air entity without additional sound ) send the patient for chest and abdominal x- ray The result of x-ray show this object in small intestine. This patient referral to imam Hussein medical city for surgical consultation.
