Vesicular Breath Sound (Normal Breathing Sound) Good air entry (normal airflow through the small airways into the alveoli) Normal breath sound heard over the peripheral lung fields Soft, low-pitched, rustling sound (like wind blowing through trees) Inspiration is longer and louder than expiration Expiration is short, soft, Best heard over most of the lung parenchyma, especially the lower lobes (DDx) if Vesicular Breath Sounds are Abnormal Decreased or absent vesicular sounds Pneumothorax Pleural effusion Atelectasis Emphysema (severe COPD) Obstructive lung diseases 2. Increased vesicular sounds Hyperventilation Increased lung compliance (as seen in early emphysema) Lung overinflation

Bronchovesicular Breath Sound • Moderate air entry (indicates partial obstruction or narrowing of airways, airflow is present but not fully normal) • Abnormal mixed vesicular and bronchial characteristics • Equal inspiration and expiration phase • Moderate pitch, intensity, and volume • **Best heard over the mainstem bronchi and upper part of the chest (normally over the major bronchi, and abnormal if heard in the periphery of the lungs) • No gap between expiration and inspiration (DDx) for Bronchovesicular Breath Sound If Bronchovesicular Sounds are Localized: 1. Early-stage lung consolidation (e.g., early pneumonia) 2. Lung collapse (atelectasis) 3. Pulmonary fibrosis (early stages) 4. Large pleural effusion (early) If Bronchovesicular Sounds are Generalized: 1. Interstitial lung diseases 2. Chronic obstructive pulmonary disease (COPD) 3. Bronchiectasis 4. Asthma (during exacerbation)
