Why can a coronary blockage look NORMAL at rest but become obvious during exercise? This is the principle behind myocardial perfusion imaging (MPI). 🟩 At rest, even a narrowed coronary artery may deliver enough blood to the heart muscle because oxygen demand is low. 🟨 During exercise, a healthy coronary artery widens, increasing blood flow by 2-4 times to meet the heart's higher oxygen needs. 🟪 A diseased artery cannot dilate normally, so blood flow increases much less. The result is a temporary mismatch in blood supply: ◻️Healthy myocardium receives much more blood. ◻️Myocardium supplied by the narrowed artery becomes relatively underperfused. Isoniazid can cause a functional deficiency of pyridoxine (vitamin B6) by forming inactive complexes and increasing its excretion. This impairs the synthesis of y-aminobutyric acid (GABA) and other neurotransmitters, leading to peripheral neuropathy, paresthesia, and, rarely,seizures. Co-administration of pyridoxine(vitamin B6) replenishes the deficiency and helps prevent these neurological adverse effects. Pyridoxine prophylaxis is especially recommended in pregnant women, malnourished patients, people with diabetes,HIV infection, alcoholism, chronic kidney disease, and those with pre-existing neuropathy. Doxycycline is the drug of choice for infections caused by Rickettsia. Rickettsia are intracellular bacteria that infect endothelial cells and cause rickettsial fever with vasculitis and systemic inflammation. Doxycycline enters infected cells and binds to the bacterial 30S ribosomal subunit. It blocks aminoacyl-tRNA attachment and inhibits bacterial protein synthesis. Rapid Eye Movement (REM) Sleep 1. During REM (Rapid Eye Movement) sleep, the eyes move rapidly beneath the closed eyelids as part of the normal sleep cycle. 2. These movements are thought to be related to intense brain activity and vivid dreaming, which is why REM sleep is often called the "dream stage". 3. MRI studies have shown that the brain is highly active during REM sleep, with activity patterns resembling those seen during wakefulness. 4. At the same time, the body develops physiological muscle atonia, meaning most skeletal muscles are temporarily paralyzed to prevent acting out dreams. 5. The extraocular muscles are spared, allowing the eyes to continue making rapid, coordinated movements despite the rest of the body remaining still. 6. REM sleep normally occurs 90–120 minutes after falling asleep and repeats several times throughout the night, with each REM period becoming progressively longer. 7. Rapid eye movements during sleep are a normal physiological phenomenon and should not be mistaken for a seizure or abnormal neurological disorder in a healthy sleeping individual.
