Orthopedic appliances are most effective when utilized during the active growth phase, specifically around the pubertal growth spurt: Class II Malocclusion: Ideal timing is during late mixed dentition or early permanent dentition (concurrent with peak height velocity) using functional appliances or headgear. Class III Malocclusion: Requires earlier interception during early mixed dentition (ages 7–9) using protraction facemask therapy before midpalatal and circummaxillary sutures mature. Q) The option to decide the correct treatment of patient by extraction of teeth Extraction is indicated under the following conditions: Severe Crowding: Tooth size-arch length discrepancy greater than 8 mm. Bimaxillary Protrusion: To allow retraction of anterior teeth and improve facial esthetics and lip competence. Skeletal Camouflage: To correct Class II or Class III skeletal malocclusions in non-growing patients where orthognathic surgery is declined. What type of anchorage (Explain)? Type of Anchorage Required: Maximum (Absolute / Heavy) Anchorage (or Cortical / Skeletal Anchorage) in the anteroposterior dimension, alongside vertical anchorage control. Explanation: Large Overjet (7 mm): Correcting a 7 mm overjet requires retraction of anterior teeth into extraction space (or significant sagittal movement). Maximum anchorage is mandatory to prevent the posterior anchor unit (molars) from moving forward (mesial loss of space) during retraction. Traumatic Deep Bite Correction: Deep bite correction requires intrusion of incisors or extrusion of premolars/molars.     Treatment plane: بعد ما عملنا التشخيص الدقيق ننتقل لخطة العلاج فمعانا ثلاث مراحل للمعالجة : ➊Remove of cause ❷Active treatment ❸ Retention نبدأ بfirst phase وهي  Remove of cause فهنا بعد ما بحثنا عن السبب في حصول diastema لازم نتخلص منه ف إذا كان السبب : ـ habits⇐ لازم نشيلها عن طريق عملhabits breaker اما متحرك ولا ثابت ةة-imacted mesiodens ⇇ نشيلھا عن طريق فتح جراحي ونشيلها ـ any pathology ⇇ نشيلها جراحي ةة.- abnormal frenal attachment ⇇نعمل له frenectomy( قص للfrenum ) ـ transient diastema⇇ ما نعمل أي معالجه لغايه مم يبزع الكانين وبيغلق diastema ـ اذا كان tooth material- arch length discrepancy ⇇ نغلق. diastema بالتقويم المتحرك او الثابت او عمل composite veneer ـ Itrogenic midline ⇇ ننتظر فتره إكتمال الexpantion وعمل تثبيت لفتره محدوده ليتغلق الsuture ونقفل diastema اما تقويميا او كمبوزت فينير او حسب امكانيه المريض وقدره الطبيب ثم ننتقل لـ seconed phase فهنا معانا Active treatment وتكون اما عن طريق 1- Removable appliance. 2-  Fixed appliance Removable appliance: نستخدم إما finger springويكون موضعه ديستل بالنسبه للسنترالات عشان يدفعههن ميزلي ويغلق المسافات او نسستخدم split labial bow ويكون بسلكstainless steel  7mm بيعمل لنا تغليق. diastema وينقص لنا overjet والبروز حق الاسنان الاماميه Fixed appliance : هنا غالبا نركب براكت للمريض ع الست الاسنان الاماميه ثم معانا عده وسائل لاغلاق diastema 1-elastics( power chain) 2-closed spring 3-M shape spring قد يكون السؤال ايش طرق اغلاق diastema عن طريقهfixed applinace تذكر له الثلاث النقاط السابقه نبدأ بال elastics( power chain) عباره عن ربلات متصله ببعضها نعملها ع البراكت حيث  نقص ع حسب الاحتياج وعدد البراكت اللي ببتركب الربلات عليه  ونركب الربلات بالبراكتات حق السنترالات  وبقيه الاسنان وتكون الربلات مشدوده وتحاول ترجع لوضعها الطبيعي وتتتولد قوه تعمل ع اغلاق الdiastema ونحتاج لتغير الربلات في حاله ضعفها وتوسعها او بستخدم closed spring بعمله بين الاترل والسنترل بالجهتين وبيولد قوه تدفع السنترالات لتغليق diastema. ثم نحتاج لـelastics( power chain) لتغليق بقيه الفراغات بين الاسنان وقد نستخدم M shape spring وهذا جهاز spring ع شكل حرفM يحتوي ع ثلاث coil. يووضع ع البراكتات حق الاسنان السنترالات وحقه الactivation يكون ب اغلاق الcoil وايضا لابد من استخدام. ( power chain) لاغلاق الفراغات بين الاسنان بالكامل شوفو الصور حق الاجهزه بالمحاضره وبتفهموا أكثر  ب إذن الله المرحله الثالثه في المعالجه هي الـRetention التثبيت وهي اهم مراحل العلاج بعد ازاله السبب ف الـdiastema إذا مفيش لها تثبيت قوي ولفتره طويل بيحدث فشل بالمعالجه 100%  وترجع الـ diastema مره ثاني فيضل التثبيت يكون ضعف الفتره العلاجيه او مدى الحياه ـ Retention عن طريق 1-lingual bounded retiners 2-hawley's retainers وقد يكون خطه المعالجه لإغلاق midline diastema ليست تقويميا ف اما نعمل cosmetic restoration بناء كمبوزت فينير او prosthesis/ crown في حاله انه في pig shape lateral واذا السن مفقوده خُلقيًا نعملremovable prosthosis لغايه 18 سنه ثم نعمل implant او fixed prosthosis هذا م استطعت فهمه وشرحه والعلم كله لله إذا في أي إضافة او تعديل معلومة او مناقشة حول أي نقطه فمرحبًا بك وسبحان من لا يسهى ولا ينسى ❤️ واتذكرو #سنرحل_ويبقى_الأثر ومعًا #للإنجاز_والتطور #اللهم_أصلح_النوايا_ما_احييتنا

    Diagnosis : التشخيص وهو أهم خطوة لنجاح أي معالجة حيث أي خطأ بالتشخيص ينتج خطأ في الخطة العلاجية وبالتالي خطأ بالمعالجة وفشل الحالة هنا بالتشخيص بنحاول نعرف الأسباب الـي تسبب بظهور diastema فلنفترض وصل مريض للعيادة عنده diastema ف أنت أول شي تأخذ االمعلومات الشخصية الاسم العمر المنطقه والعرق الخ .... فمن الاسم واللقب والمنطقه ومن نظرتك للأب إذا موجود هل عندة نفس المشكله فقد تتنبأ بأن المشكلة racial وأن الجين والعرق هم السبب في diastema وقد تتشوف هل السبب عاده مص الاصبع او غيرها ثم نأخذ القصه المرضيه ومن متى حصلت المشكله وهل قد خضع المريض لمعالجه تقويميه سابقه فمن هنا بتتنبأ بعده أشياء منها هل المريض متعاون معك أو لا وممكن تعرف سببdiastema إذا كان خضع للمعالجه تقويميه والدكتور اللي قبلك عمل له expansion للفك العلوي وتتنبأ لهذا الأمر هنا يفضل تستكمل التشخيص حقك فقد يكون مصاحب للمشكله عدم بزوغ الكانين او عندناabnormal frenum. فهنا تعمل له فحص خاص بالfrenal attachment ويسمى blanch test. تعمل سحب للشفه العلويه بتجاه الخارج وبتلاحظ ضهور thick and fleshy frenum. وبتلاحظ بياض بمنطقه incisive papilla. فهذا يدل ان السبب في diastema هو. Abnormal frenal attachment فنحن م نكتفي ب بالكشف ع سبب واحد فقد يكون مترافق مع اكثر من سبب من الاسباب المذكوره سابقا فنحن نستكمل التشخيص ونكتب كل شي وجدناه عشان نكتب خطه علاجيه دقيقه... فلنفترض إنه المريض الكانين حقه مش بازغ فهنا أنت بتحتاج لعمل أشعه فبتشوف من خلالها موضع الكانين وهو ب اي ممستوى من مستويات البزوع فلو الكانين نازل بمكانه الطبيعي ف انت تطمن اهل المريض ان المشكله طبيعيه وانه اول مما يبزع الناب بتتقفل المسافه تلقائي فقط ينتظرو ريثما يبزع الناب والامور ماشيه حلااااوه طيب نبقي بالاشعه ونكتشف بقيه الاسباب نبحث عن وجود اي pathology بالمنطقه مثل cyst او tumer. نشوف هل في mesiodens unerupted ( سن زائده بين السنترالات ) فإذا وجد اي شيء من المذكور فتسجله عشان تضيفه للخطه العلاجيه فيما بعد وايضا تشوف هل في منطقه radiolcent بين السنترالات فهذا ييدل ع انه معانا abnormal frenal attachment واذا مش موجود اي سسبب من الاسباب نشوف حجم السنترل. فالطبيعي حدود 8 mm اذا لقيناها اقل من كذا فمعناه إنه المشكله كانت بحجم السنترل اللي تسببت بوجود diastema ونأخذ طبعه للمريض ونفعل cast analysis عشان نشوف طول القوس السني بالنسبه ل حجم الاسنان فقد تكون السن حجمها طبيعي لكنarch كبير وقد يكون الarch طبيعي لكن الاسنان حجمها صغير. فنجمع الاسباب التي كونت لنا diastema ونرتبها بشكل متسلسل ونعمل خطه علاجيه محكمه فبختصار التشخيص تجميع معلومات بتسلسل وترتيبها لعمل خطه علاجيه محكمه فلازم تبحث عن كل الأسباب هنا قد يأتي لك سؤال اذكر. او عدد وسائل التشخيص لمريض diastema فبيكون إجابتك بالتسلس 1ـ من القصه المرضيه نتنبأ. بالآتي: ⓵إنه الdiastema ناتجه عن عامل عرقي ⓶ خضوع المريض لمعالجه تقويمية وعمل expantion ⓷وجود اي عادات سيئة مثل مص الأصبع وغيرها حيث القصه المرضيه قد تكون عامل مساعد للتشخيص وقد لا تكون 2ـ سريريا نشوف : - هل الكانين موجود او لا ونتأكد من وجود الكانين بالنظر لفم المريض او عمل اشعه اذا كان ما زال داخل العظم السنخي - ايضا نفحص الfrenum عن طريق blanch testونتأكد منه بالاشعه 3ـ نأخذ كاست ونعمل تحليل لمعرفه التناسق بين طول الفك وحجم الاسنان ونشوف فين المشكله 4ـ نأخذ أشعه ومن خلالها بنشوف: ـ هل الكانين بمكانه وبزوعه طبييعي ام بيبزع بغير مكانه او بيبقي impact ـ هل معاناradiolcent بين السنترالات لنتنبأ بوجود abnormal frenum - هل معانا أي pathogen بالمنطقه او وجود mesiodens impacted ـ بنشوف هل الmaxillary suture مفتوح نتيجه ال expantion في الفك وكل وسيله تشخيص مكمله لختها فما نكتفي بالقصه المرضيه او ببالفحص السرير او الاشعه او الكاست لازم نتسلسل بالاربع الوسائل

    Management of midline diastema: Distema refers to anterior midline spacing between the two maxillary centeral incisors طبعًامعالجتها سهله لكن صعب الretention Cause of midline diastema? ⓵ Transient malocclusion ② Tooth material arch length discrepancy: ③ Abnormal frenal attachment: ④ pressure habits: ⑤midline pathology: ⑥Iatrogenig midline: ⑦ Racial prdispostion نبدأ نشرحهم بالتفصيل ⓵ Transient malocclusion : يعتبر السبب الأول من أسباب الmidline diastema وهذا يحدث لعده أسباب وعاده لايحتاج لتدخل علاجي انما تتغلق الdistema مع بزوغ الاسنان الدائمه Transit diastema occurs in some case ? يعني الحالات اللي يكون معانا transient diastema ( وقد يكون السؤال بطريقه أخرى⇦ماهي المراحل اللتي يحدث فيها transient diastema) وِڪذا تكون الإجابه⇩ ❶ in deciduous dentition ( generalizedspacing ) وهذا الspace يكون طبيعي ويساعد في رص الأسنان الدائمه لأنه حجمها اكبر من الاسنان اللبنيه وتحتاج إلى مسافه عشان ترتص بالشكل الطبيعي فوجود الفراغات بين الاسنان اللبنيه يكون عامل للتخلص من malocclusion ❷ transient diastema occurs during mixed dentition وهذا النوع يكون مرتبط مع بزوع الكانين الدائم حيث اول ما يبدأ الكانين يتحرك وسط الفك عشان يبزع يتسبب بضعط ع الروت حق lateral incisor وتنزاح mesially وتتسبب بضغط على الروت حق centeral incisor وينزاح الروت ايضا ميزيلي اما الكروان حق السنترال واللاترل بينزاحو دستلي ع( عكس انزياح الروتات) وبهذا الطريقه يتكون معانا transient diastema وهذه المرحله نسميها ugly duckling stage تحدث بشكل طبيعي وعند جميع الأطفال وما نفعل لها اي معالجه يعني إذا صادفت حاله مرضيه وعنده نفس المشكله هذه والكانين عاده ما بزغ فما تبدأ ب اي معالجه تقويمية انتظر لما يبزع الكانين فهو بيغلق الdiastema تلقائي لانه يتحرر الضغط ع الروتات ويرجع الضغط في الكروان حق اللاترل وينتقل الضغط للكروان حق السنترل وتتغلق المسافه دون تدخل علاجي اما اذا بزع الكانين وفي diastema. فهنا نبحث عن السبب اللي كونها ونبدأ نعمل ازاله للسبب وعمل معالجه تقويمية ② Tooth material arch length discrepancy: السبب الثاني لـحدوث الـ. diastema المعروف والطبيعي إنه طول الarch يكون متوافق ومتساوي مع حجم الأسنان عشان يحصل ارتصاف للأسنان بالشكل الطبيعي فهنا يكون اما : 1- طول الarch طبيعي لكن الاسنان microdontia. يعني حجم الاسنان صغير فتبزغ للarch ويبقي في فراغ بينها 2- او تكون الاسنان بالحجم الطبيعي والarch يكونmacrognathia يعني الarch.طويل 3-قد يكون بسبب missing teeth 4- وقد يكون بسبب قلع الاسنان ويحصل drifting للأسنان المجاوره مثلا قلعت اللاترل لسبب من الأسباب قبل موعد بزوغه وحدث بزوغ للسنترل لمحل اللاترل ويتكون diastema ③ Abnormal frenal attachment: أولا الـ. Frenal attachment هو عباره عن fibrous connective tissue يكون على الalveolar ridge ف اول م يبزع الكانين تبدأ frenal attachment بالمهاجره apically وتتغلق الdiastema بين الستترالات فإذا لم يحصل لها انزياح ناحيه الـ apical. تتسبب بضهور midline diastema ④ pressure habits: المريض لما يكون عنده عادات سيئة مثل thumb suking او tongue thrusting يتسب بdiastema بين السنترالات وproclnation مع general anterior spacing ⑤midline pathology: وجود اي pathology مثل cyst او tumor فيsoft or hard tissues يتسبب بعمل. Midline diastema ايضا وجود mesiodens غير بازغه بين الروتات حق السنترالات قد تسبب لنا diastema ⑥Iatrogenig midline: هنا. diastemaتكون بسبب الطبيب يعني تتكون نتيجه إجراء علاجي وتكون خلال عملrapid expansion. في الفك العلوي وتكون مؤشر مهم لحدوث opening of intermaxillary suture ⑦ Racial prdispostion: تكون. الـ diastema سائده عند أبناء بعض المناطق او في بعض العائلات
