🔴The South African Flag Sign on ECG 🇿🇦⤵️ 🔹A unique pattern seen in mid-anterior (high lateral) MI due to first diagonal (D1) occlusion. D1, a branch of the LAD, supplies part of the LV's anterior wall & basal anterolateral areas. 🔹The South African Flag Sign is characterized by: ❤️ST-segment Elevation: in leads I, aVL, and V2 ❤️ST-segment Depression: in lead III (and often in other inferior leads) 🔹This pattern resembles the green Y-shape of the South African flag when visualized on a standard 12-lead ECG display. 🔹Culprit Coronary ❤️First Diagonal Branch (D1) Occlusion: Typically results in isolated ST elevation in leads I and aVL. ❤️Left Circumflex Artery (LCx) Occlusion: May cause ST-segment elevation in leads I, aVL, and V5-6. 🔹Clinical relevance: ❤️Recognizing the South African Flag Sign on ECG is crucial, as high lateral STEMIs are often missed due to non-contiguous ST-segment elevations. ❤️Timely identification ensures prompt intervention, reducing the risk of significant cardiac damage

مهم جدا جدا كل طبيب يعرف هاي الستيبس.. و سيناريو جدا شائع بالامتحانات. ✅ Approach to Sudden-Onset Dyspnea in the Emergency Department When a patient presents with acute shortness of breath (SOB), your priority is to rule out life-threatening conditions. Most are potentially fatal if missed. 🔴 Cardiac Causes 1. Myocardial Infarction (MI) 2. Arrhythmias 3. Acute Decompensated Heart Failure (Pulmonary Edema) 4. Aortic Dissection – Suspect in any patient with sudden chest pain + high BP 5. Cardiac Tamponade 🔴 Respiratory Causes 1. Pneumothorax – Especially in trauma patients or young, tall males 2. Pulmonary Embolism (PE) 3. Acute Exacerbation of Asthma 4. Acute Exacerbation of COPD 5. Hypersensitivity Pneumonitis – Due to exposure to cleaning agents, birds, animals 6. Airway Obstruction (Foreign Body) – Common in children 🟢 Other Important Causes 1. Anaphylaxis – Especially in summer (insect stings, meds) 2. Organophosphate Poisoning – Common in agricultural area. 🟨 Initial Clinical Assessment 🔹 Is the patient in respiratory distress? Look for: Nasal flaring Use of accessory muscles Cyanosis Intercostal/subcostal retractions ➡️ Start O₂ immediately ➡️ Begin rapid history + focused physical exam 🔍 Focused Examination Vital signs Chest exam → rule out pneumothorax _Cardiac exam _Lower limb exam → DVT (suspect PE) _Skin exam → rash (anaphylaxis, drug reaction) 🗒️ Focused History Ask: What was the patient doing when SOB started? Any previous similar attacks? _Provoking/relieving factors? _Associated symptoms: chest pain, fever, cough, wheeze? _PMH, Medications, Surgical history _Family & Travel history _Allergen/chemical exposure?

A 45-year-old man with a history of persistent atrial fibrillation presents to the dermatology clinic with concerns about a gradual bluish discoloration of his face and hands that has developed over the past six months. He reports no pain, itching, or other associated symptoms. Dx? Cause?

الكل يعرف الجوكر مال arrythmia Amiodarone (cordarone) زين خل نعرف شنو تاثيره ع thyroid gland وليش مرات يسوي hypo و مرات hyper ... مثل منعرف هو يحتوي على كمية هائلة من اليود. كل جرعة 200mg من Amiodarone تحتوي على 75mg من اليود. هذا اليود العالي يؤثر بشكل مباشر على تصنيع هرمونات الثايرويد. ليش Hypothyroidism؟ 🧠 الفكرة الأساسية: Wolff–Chaikoff Effect عندما تزداد كمية اليود داخل خلايا الغدة الدرقية→يتم إيقاف تصنيع الهرمونات الدرقية مؤقتاً كـ"استجابة ذاتية دفاعية". هذا الانقطاع يسمى: Wolff–Chaikoff effect. يستمر عادة 10-14 يوم فقط. بعدها، ترجع الغدة للإنتاج الطبيعي بسبب ما يسمى: Escape phenomenon. 📌 في بعض المرضى، لا يحدث الـ Escape! خاصةً في مرضى autoimmune thyroid disease. النتيجة؟ استمرار الـ Wolff–Chaikoff effect → Hypothyroidism مزمن. 3. لماذا Hyperthyroidism؟ 📌 بسبب طريقتين: 🅰️ أولاً: Jod–Basedow Effect إذا كانت الغدة تحتوي على nodular goitre أو Latent Graves’ disease: الغدة تكون غير قابلة للتنظيم الذاتي. زيادة اليود → تحفز الإفراز → Hyperthyroidism. 🅱️ ثانياً: Destructive Thyroiditis Amiodarone يسبب أذية مباشرة لخلايا الغدة→تطلق الهرمونات المخزونة→Hyperthyroidism مؤقت.

بالguidelines القديم لل ESC كان انه اذا ال AF صارلهة يومين او اقل نحولهة الى sinus rhythm سواء electrical or pharmacological cardioversion هسة بالكايدلاين ال2024 صار يوم واحد فقط 😱.. يعني اقل من يوم نحاول نرجعة اكثر من يوم نبقي rate control
