What is the primary mechanism of action of Acetazolamide and which part of the nephron does it primarily act upon?
💊Drug of the day Acetazolamide 👉It is a Carbonic Anhydrase inhibitor. 👉Used in Glaucoma: In this situation, the optic nerve is damaged. 👉Used as a weak or adjunctive diuretic. 👉It works on the Proximal Convoluted Tubule of the kidney. 👉It acts by blocking the enzyme Carbonic Anhydrase. 👉In Glaucoma, it lowers the pressure of Aqueous humor. 👉Helpful in Mountain Sickness. It increases the breathing rate in altitude sickness and stops the alkalosis. 👉Side effects are Metabolic Acidosis and Hypokalemia. 👉It makes the urine alkaline; just because of this, the risk of stone can occur. 👉It excretes bicarbonates through urine. 👉Acetazolamide inhibits the generation of bicarbonate ion in the ciliary epithelium. 👉Carbonic Acid: H2CO3 👉Due to inhibition of Carbonic Anhydrase, hydrogen ions are not available for exchange with Sodium. So, sodium is not reabsorbed and is eliminated. Along with it, water is also eliminated. Diuresis produced by acetazolamide is accompanied by loss of bicarbonate and potassium. 👉It also has an anti-thyroid and anti-convulsant effect. 👉It is eliminated by the kidney in an active form.
During paracetamol metabolism in the liver, what highly reactive and toxic intermediate is formed?

abb se inke quiz bhi saath saath kiya krenge
💊Drug of the day Paracetamol 👉Used as a simple analgesic in migraine during attack. 👉Paraminophenol Derivative. 👉Another name is Acetaminophen. 👉Chemical name is N-acetyl-para-aminophenol (APAP). 👉Formula is C8H9NO2. 👉Used as Antipyretic & Analgesic (NSAID). 👉It blocks the enzyme COX in the brain which is responsible for Prostaglandin. 👉It acts on the thermostat of the body i.e., Hypothalamus to lower the body temperature. 👉Metabolism takes place in the Liver. 👉During its metabolism a toxic by-product forms named— NAPQI>>N-acetyl-p-benzoquinone imine. 👉In normal dose, the Glutathione of the liver neutralizes this toxic substance. 👉Overdose of Paracetamol causes hepatotoxicity (liver damage). 👉Antidote>> N-acetylcysteine. 👉Generally, its production starts from the Phenol or Nitrobenzene. 👉Paracetamol is Achiral. 👉Benzene ring is present in its structure. 👉Paracetamol does not affect the platelet aggregation.
