In endodontics, a patient’s age can provide valuable clues about the root canal anatomy you may expect even before instrumentation begins. As patients age, the continued deposition of secondary and tertiary dentin leads to a reduction in pulp chamber volume, changes in access to canal orifices, and progressively narrower, more frequently sclerotic canals. At the same time, the clinical priorities also change: ☑️ in younger patients — preserve tooth structure and avoid unnecessary dentin removal ☑️ in middle-aged patients — account for isthmuses, oval canal areas, and complex internal anatomy ☑️ in patients over 40 — safely negotiate and shape narrow, sclerotic canals Importantly, there is no universal instrumentation protocol. Instrument selection, the extent of apical enlargement, additional canal-wall preparation, and the irrigation strategy should all be based on the actual anatomy of the individual tooth, with age serving only as one clinical guide. 📚 In the carousel, we review practical recommendations from the work of Michael Solomonov and co-authors for patients aged ≤20, 20–40, and ≥40 years. Today, we’re reviewing a clinical case by Pierpaolo Cortellini — treatment of tooth 11 with a deep intrabony defect in a patient with severe generalized periodontitis 🩸 The patient presented with severe inflammation, pathological tooth migration, hypermobility, and deep periodontal pockets. The first stage focused on cause-related periodontal therapy and inflammation control. The decision to proceed with regenerative treatment was made only after re-evaluation. During surgery, a deep, wide, 10 mm one-wall intrabony defect was identified. Cortellini used a combined regenerative approach with preservation of the soft tissues and achievement of primary wound closure. ☑️ Outcome: at 1 year, there was a 7 mm gain in clinical attachment and complete resolution of tooth mobility. At 6 years, the tooth had completely and spontaneously realigned, and the result remained stable. Predictable aligner treatment begins not with the digital setup, but with an understanding of biomechanics 🦷 Even carefully planned digital tooth movements are not always fully expressed clinically.
