ACUTE CASE OF COUGH A 17 years girl Chief complaints:- Cough since one week Details of chief complaints:- She entered in clinic while coughing, I carefully listened she coughs in either three or four paroxysms. Cough more during sleep. On examination:- A big swelling of lymph node at left cervical region bellow the lower jaw (submandibular lymphadenitis). Skin is cold sweaty to touch. RUBRICS FROM SYNTHESIS REPERTORY :- 1.COUGH - paroxysmal - consisting of - three coughs - or four coughs 2.COUGH - sleep - during - agg 3.EXTERNAL THROAT - inflammation - cervical glands 4.PERSPIRATION - cold Rx:- Belladonna 0/1 (LM1) 3 doses (one dose at interval of 10 minutes ) Followed by bid for 1 week. A CHRONIC CASE OF OA KNEE JOINTS This is a very short but wonderfully intresting case to elaborate the prescription based on singular peculiar rubric. A 47 years female patient. Chief complaint : Pain in right or left knee joint alternating side since 2 years Patient had consulted orthopedic surgeon who advised surgical treatment (joint replacement) after seeing x-ray of knee joint ( showing gross osteoarthritic changes) so patient consulted me for homoeopathic treatment to avoid surgery. Details of chief complaint: Patient was limping while walking due to pain in knee joint (presently left knee joint) There is no any other complaints , and also no any specific details of complaint. Generals :- Chilly patient RUBRICS FROM SYNTHESIS REPERTORY : EXTREMITIES - LIMPING - pain in knee, from (Single remedy rubric) Author reference of this rubric is also very authentic ( HAHNEMANN Samuel - Materia madica pura) The indicated remedy is Spigelia Only available physical general of case is chilly patient, and Spigelia is also chilly medicine. A CHRONIC CASE OF ALLERGIC RHINITIS WITH SINUSITIS A 48 years male patient. Chief complaints : Coryza nose obstruction pain and heaviness in nasal sinuses. Since many years almost 10 to 15 years on allopathic medicines. Details of chief complaints : Nose Obstruction at night Complaints of coryza < change of weather, cold weather, cold food and drinks Coryza associate with sensation of fullness and congestion in ears. Generals :- Hot patient Sleeplessness after waking in between sleep. Mind:- Patient asked some questions curiously like, Does itching occur due to BP tablets? Associated complaints:- Itching red rash on skin since 1 week, it occurs infrequently. Hypertension on allopathic treatment since 2 years. Chronic piles with constipation since 4 to 5years, PR bleeding occurs infrequently, recently it happened somedays ago. PAST History :- Kidney stones, two times. RUBRICS FROM SYNTHESIS REPERTORY : 1. NOSE - obstruction - night 2. NOSE - sinuses, complaints of 3. NOSE - hay fever (allergic rhinitis) 4. EAR - fullness, sensation of (concomitant) 5. GENERALS - stones in organs, formation of (It is not present complaint still considered in rubric totality as a diathesis, because stone formation happened two times in past so it shows stone formation tendency, we considered diathesis as a individualising factor in case totality ) 6. SLEEP - sleeplessness - waking agg, after 7. MIND- curious After repertorisation we get three leading medicines LYCOPODIUM, CALCARIA, SEPIA. Which covers all the selected rubrics but patient is hot so we preferred LYCOPODIUM and ruled out CALCARIA and SEPIA as chilly medicines. Rx :- LYCOPODIUM 0/1 bid 2 weeks. Follow up after two weeks :- Significant relief in nasal complaints with ear complaints also. Sleep good No constipation Tingling in feet also reduced Itching aggravated so he took some allopathic medicine (so strictly advised don't interfere any allopathic medicine) In next follow up after Lycopodium 0/2 :- Itching also significantly relieved with overall improvement in all complaints, this time BP is at lower side 110/70 so allopathic antihypertensive medicines tapered to half. A CHRONIC CASE OF DIABETIC NEPHROPATHY 60 years male patient. Chief complaints:- Edematous swelling on legs Breathlesness Complaints since 6 months. Known case of Diabetes on allopathic treatment since 6 years. Lab Investigation Details:- Blood Urea : 72.70 mg/dl Urea nitrogen : 33.97:mg/dl Sr creatinine : 1.73 mg/dl USG abdomen:- Billaterly small kidneys with relatively bulky cortices , chronic diabetic nephro/ medical renal parenchymal disease, simple left renal Cortical cyst (Bosniak category one), mild diffuse urinary bladder wall thickening with insignificant post - void residual urine (5cc). Grade 1 Hepatic stenosis Chronic calculus cholecystitis with multiple small gallstones (2.5mm) Mild prostato megaly with minimum prostatic calcification. Mild gaseous bowel distension. Details of chief complaints : - Edematous swelling on legs since 6 months but not responding to conventional allopathic treatment. Aggravation after travelling in bus, hanging down lower limbs on seatting chair. Difficult respiration < walking and ascending Generals :- Hot patient Sleep position:- patient explained that he sleeps on right side then turn on left at 2am. RUBRICS FROM SYNTHESIS REPERTORY : 1. EXTREMITIES - swelling - legs 2. GENERALS - swelling - puffy,edematous (grand generalisation, pathological generals) 3. GENERALS - hang down, letting limbs - agg ( grand generalisation at the level of modalities) 4. RESPIRATION - difficult - walking - agg 5. RESPIRATION - difficult - ascending - agg 6. KIDNEYS - complaints of kidney (USG abdomen finding) 7. SLEEP - position - side,on [ Boger's concept of grand generalisation Or pathological generals (applied in above mentioned rubric no. 2 and 3) We apply Boger's this concept when medicines listed under particular part rubrics when not matching with overall totality of symptoms/rubrics. for example in above case : Particular rubric : Extremities - swelling - legs - hang down, letting leg : lathyrus ( Single remedy rubric) But Lathyrus does not match with overall totality like edematous puffy swelling, , complaints of kidneys, breathlesness agg. walking and ascending, generals hot patient etc. so we considered genral modlity ( Rubric no.3) Similar thing about particular part rubric (Extremities - swelling -leg edematous ) You can cross check this particular in above way so we considered General pathological rubric no.2.] After repertorisation we get two leading medicines SULPHUR and NUX VOMICA which covers all the selected rubrics but patient is hot so we prefered SULPHUR and ruled out NUX VOMICA Rx:- SULPHUR 0/1 bid two weeks. Follow up - within two weeks edematous swelling on legs relieved almost completely, on examination there is no edematous swelling on legs. Patient told that there is no swelling on legs even after long traveling. Also significant relief in breathlessness . Asked patient to give regular follow ups.
