Pediatric age patient presented with his mom to ED she said that her child ingested a large amount of paracetamol, the pt was healthy with no signs of any toxicity on presentation, and they observed the pt for 4 hours with no any change in health no signs and symptoms of toxicity, the mom then mentioned that she found some of the drugs was spilled on his clothes and in the floor, what is next? A. Reassure B. Watch for another 4 hours✅ C. Give antidote 4 year old boy brought by his mother she said he drink a bottle of paracetamol after 4h of monitoring the child examination was normal the mother said the bottle has little in it before he drunk it and she found some in his clothes, There was no investigation ordered, What's the next step? A - Give N- acetyle cysteine B- Monitor for more 4 hours more✅ C- discharge home with instructions The safest option is to monitor the patient for another 4 hours. Basically we should draw blood at 4 hrs to see the level if it is toxic or not, if the missing option is to measure serum APAP then it is the right answer, otherwise with these options choice B is the closest answer. In phase 1 (0.5-24 hours after ingestion), patients may be asymptomatic or report anorexia, nausea or vomiting, and malaise.

A child with mild respiratory infection increase symptoms & fever, develop respiratory distress. X-ray shows lobar infiltration, CBC shows low HB and high Retic. A- SCA ✅ B- Mycoplasma pneumonia C- pneumococcal pneumonia This Q came in my exam, there was also chest pain, systolic and pulmonic murmurs.

A child came with severe dehydration and started on IV fluid. After 8 hours started to sieze • Serum sodium came and found to be 162 Most likely cause for his seizures A-Cerberal edema ✅ B-Central pontine neurolysis C-Renal failure D-Metabolic acidosis I believe this is Hypernatremic dehydration + development of seizure in such rapid time ( 8 hours) supports cerebral edema.
