🎓 MEDICAL EDUCATION: Iran vs USA 📚 Pathway & Graduation • Iran: • Students enter medical school directly after high school. • Internship lasts ~18 months and includes Internal Medicine, Surgery, OBGYN, Pediatrics, and rotations in subspecialties. • After graduation, physicians are licensed general practitioners who can work immediately without residency (except for certain specialties). • USA: • Students complete a bachelor’s degree, then medical school. • After graduation, they must complete residency to practice independently (no general practice license after medical school). • No single long rotating internship like Iran; specialization begins immediately. 🧪 Focus & Training Style • Iran: Broad practical training during internship; emphasis on clinical service, inpatient management, and hands-on procedural work early. • USA: Strong early focus on standardized exams (USMLE), research, and academic evaluation; specialization-focused training begins sooner. 💰 Education Cost • Iran: Government subsidizes medical education; low tuition but lower physician salaries after graduation. • USA: Medical education is very expensive, producing high student debt; physician salaries are higher after residency. 🩺 Workforce Outcome • Iran: Produces large numbers of general physicians serving cities and rural areas. • USA: Produces highly specialized physicians; fewer general practitioners due to specialization incentives.
IRAN vs 🇺🇸 USA – Healthcare System 🏥 Structure & Access • Iran: Largely government-funded & insurance-supported. Essential care is widely accessible at low cost. • USA: Mostly private and insurance-driven. Care varies by insurance type; advanced but costly. 💳 Cost & Coverage • Iran: Subsidized health services and medications; public insurance covers much of the cost. • USA: Employer-based and private insurance dominate; high out-of-pocket expenses even with insurance. 👨⚕️ Primary Care & Referrals • Iran: Patients commonly access specialists directly, which boosts access but can overload hospitals. • USA: Primary care physician (PCP) acts as coordinator; referrals are often required. 💉 Technology & Standards • Iran: Strong public health & affordable medications; variable advanced technology across cities. • USA: Heavy investment in research, medical technology, and electronic systems → high quality, high cost. 🚑 Patient Disposition & Social Services • Iran: Limited structured social work for discharge planning, rehab placement, or nursing home transfer. • USA: Strong interdisciplinary discharge planning involving social workers, case managers, rehab, and home-care teams. ⚖️ Culture & Legal Aspect • Iran: Physician-centered decisions; family involvement is common; fewer malpractice lawsuits. • USA: Patient autonomy is a priority; informed consent is mandatory; litigation risk influences practice.

🚨Clarification: Recently, there was a news report from state media regarding the resumption of J and F visa processing. This change occurred due to a new order that lifted the previous worldwide visa issuance suspension. The new policies now allow for the continuation of visa processing, but with more detailed screening. It is important to note that this does not exempt Iran from the initial travel ban order. https://www.state.gov/releases/office-of-the-spokesperson/2025/06/announcement-of-expanded-screening-and-vetting-for-visa-applicants/
🇺🇸📢 Important Update for Iranian Physicians Planning to Immigrate to the United States (June 2025) Due to the June 9, 2025 U.S. presidential proclamation, all nonimmigrant visa routes (including J‑1 and H‑1B) are currently suspended for Iranian nationals outside the U.S., unless they hold a valid visa issued before June 9 or qualify for a rare national interest exemption (NIE). As a result, many Iranian medical graduates (MDs) — especially those without U.S. clinical training or licensure — are now unable to enter the U.S. via traditional residency pathways. ⸻ 🔍 What Does This Mean for Iranian Doctors? Most common entry paths for medical graduates — such as ECFMG-sponsored J‑1 visas for residency or H‑1B clinical positions — are no longer viable for applicants currently in Iran. However, certain immigrant visa categories remain open, particularly for those who qualify through national interest, academic excellence, or employment-based green card routes. ⸻ 📚 Current U.S. Immigration Options for Iranian MDs (June 2025) shown in the table. ——- 🧭 Practical Recommendations If you’re an MD in Iran without U.S. residency or license, here’s what you can do now: 1. Focus on academic/research/public health experience — especially internationally collaborative work. 2. Build a strong EB-2 NIW case (non-clinical), emphasizing how your contributions serve U.S. national interests. 3. Avoid relying on J-1/H-1B until/unless the travel ban is lifted. 4. Explore third-country options or research fellowships that may lead to later U.S. immigration. 5. Consult with experienced U.S. immigration attorneys who handle NIW/EB-1 filings for medical professionals. ⸻ ➖Final Note This situation is fluid, and policy changes are possible in the coming months. However, as of now, Iranian physicians cannot apply for U.S. residency via J‑1 or H‑1B unless they were already holding valid visas before June 9, 2025. The best available options are through self-petitioned immigrant categories (EB-1A/EB-2 NIW) based on research, education, or public health impact. 👁We will continue to monitor developments and share verified updates. Stay informed. Share this with your colleagues.
