Which are shown in the picture above. >Applying the criteria step-by-step to our patient's presentation, it gives results 10/10. Final Diagnosis: ​ Acute Appendicitis (with a high-probability Alvarado score of 10/10, reinforced by localized peritoneal irritation and normal scrotal examination findings that successfully ruled out referred pain from epididymal colic). Management Plan: بما أن التشخيص صار واضح ومؤكد الشغل لازم يصير سريع؛ >​Stabilization: Immediate NPO"nothing by mouth" status and IV fluid resuscitation. >​Medication: Empiric broad-spectrum IV antibiotics and adequate analgesia. >​Surgery: Urgent appendectomy (laparoscopic or open) to prevent perforation and peritonitis.

Examination & Labs >Vital signs: All normal except for Temperature: 37.8 C° [⬆️] HR: 100bpm [⬆️] >Abdominal Palpation: The abdomen is not distended. On inspection, the patient is motionless and appears in pain. On percussion, there is marked percussion tenderness in the RLQ. On palpation, the abdomen is soft but reveals involuntary guarding and severe localized tenderness at McBurney's point, with positive rebound tenderness. >Normal Scrotal Exam scrotal examination was performed to rule out referred pain from epididymo-orchitis or torsion. The examination was completely unremarkable. Both testes and epididymides are normal in size, non-tender, and there is no scrotal swelling or erythema. [ it’s done for male patients only, Because the appendix and the epididymis share the same embryonic nerve supply (T10-L1). Therefore, inflammation in the epididymis can cause referred visceral pain to the RLQ, presenting as an 'epididymal colic' before localizing to the scrotum.] >Lab ...CBC WBC: 13,500 cells/mcL. (Leukocytosis) ​Neutrophils: 82%. (Neutrophilia / "Shift to the left") ...(GUE) Normal ,Clear, with no pus cells (pyuria) or red blood cells (hematuria). ​[Note: A normal GUE is crucial in this case to rule out urinary tract infections, renal/ureteric colic, or a pelvic appendix irritating the bladder].

#طوارئ 6 شاب عمره 22 سنة، جابوه لطوارئ وهو يتلوى من وجع بطنه، وصلنا للمريض وجان متمدد على السرير ثاني ركبتيه و ساكن ما يتحرك لان اي حركة جانت تزيد عنده الالم. هستوري الألم: بدا عنده الوجع قبل 8 ساعات، بالبداية جان وجع مبهم داير مداير السرة (Visceral pain)، وراها زحف ونزل جوة على اليمنى (Localization). وياها المريض نفسه مسدودة عالأخر (Anorexia) ولعبان نفس، وصار عنده vomiting مرة وحدة. هذا جان كلام المرافقين الحرفي .

Compliant Chest and Abdominal Walls: In pediatric patients, the ribs and abdominal walls are highly flexible and compliant. Upon high-energy impact, they do not fracture to absorb the kinetic energy; instead, they bend inward, transmitting the entire force of the impact directly to the underlying visceral organs. Solid Organ & Vascular Injury: This direct transmission of force resulted in catastrophic internal injuries, specifically a severe laceration/rupture of solid organs (such as the liver or spleen) and/or a major disruption of the primary abdominal vasculature (abdominal aorta or inferior vena cava). Internal Hemorrhage: Due to the massive rupture of these highly vascular structures, the patient's entire circulating blood volume rapidly extravasated into the peritoneal cavity within minutes. This rapid accumulation of blood was the direct cause of the acute abdominal distension observed immediately post-injury performing continuous Cardiopulmonary Resuscitation (CPR) for 20 minutes. Unfortunately, the patient remained unresponsive, and death was pronounced. 》The patient suffered a traumatic cardiac arrest secondary to severe hypovolemic/hemorrhagic shock Because the massive internal hemorrhage had completely depleted the intravascular volume, the heart was entirely empty (pumping on empty), Consequently, manual closed-chest compressions were mechanically ineffective; without sufficient blood volume returning to the heart, compressions could not generate the necessary stroke volume to deliver oxygenated blood to the brain and myocardium, rendering the CPR unsuccess Immediate abdominal distension post-trauma is an absolute clinical emergency indicating massive hemoperitoneum. In a profoundly hypotensive or arrested patient, conventional CPR alone is futile without aggressive volume resuscitation. Unfortunately it was Non-survivable injury.💔💔 "we can conclude that the child arrived too late, already clinically dead with cardiac arrest. The blood transfusion and IO access were merely attempts to buy time to get him to the operating room, rather than the definitive treatment. However, since the child was already dead, all of these interventions would ultimately be futile."
