A This patient's development of tremor, psychomotor retardation (bradykinesia), and decreased arm swing shortly after an increase in risperidone dose is suggestive of antipsychotic-induced parkinsonism.   Adding an anticholinergic antiparkinsonian medication when the antipsychotic is working well and there is concern for destabilizing the patient with a dosage reduction or switching to a new antipsychotic (as in this patient). The anticholinergic antiparkinsonian medication benztropine is commonly used.

  • A 52-year-old woman with a history of schizoaffective disorder, depressive type, is brought to the office by her husband for follow-up.  The patient was hospitalized last month due to worsening auditory hallucinations that were telling her to kill herself.  She responded to an increased dose of risperidone and was discharged a week later.  The patient reports feeling relieved that the voices telling her to kill herself have gone away since her antipsychotic dose was increased.  Her husband also reports that she is much less agitated since the voices have subsided, but he is concerned that she smiles less and seems slowed down.  He is worried that she is becoming depressed.  On examination the patient is alert, calm, and cooperative.  She has a mild tremor and does not swing her arms when she walks.  Her affect appears sad.  She denies any auditory hallucinations or thoughts of wanting to die.  Which of the following is the most appropriate pharmacological management for this patient's condition?

  • A 55-year-old man with a history of severe hypertension is admitted to the ICU for management of a hypertensive emergency. He is started on an intravenous antihypertensive infusion. Twelve hours later, the patient becomes disoriented and develops generalized seizures. His skin appears flushed, and laboratory studies show a markedly elevated serum lactate level. Which of the following best explains this patient’s findings?

  • A 10-day-old male newborn is brought with 12 hours of weakness, spasms, poor feeding, and difficulty opening his mouth. No fever or illness. Born at home (39 weeks, uncomplicated), mother unvaccinated. His mother reports the baby may have ingested raw honey 5 days ago. Exam: generalized stiffness, twitching, soft fontanelles. Which organism is most likely?

  • In this case: Ultrasound shows hypoplastic nasal bone: Indicative of trisomy 21 (Down syndrome). Low α-fetoprotein: Can suggest an increased risk for Down syndrome. Increased β-hCG and inhibin A: highly associated with Down syndrome. So the correct answer is: Single transverse palmar crease 👉Other choices: ⚡Ambiguous external genitalia: Related to disorders of sexual development, such as congenital adrenal hyperplasia. ⚡Microphthalmia: This is seen in conditions like trisomy 13, Patu syndrome. ⚡Extremity lymphedema: More associated with Turner syndrome. ⚡Meningomyelocele is a neural tube defect associated with folic acid deficiency prior to pregnancy. ⚡Clenched fists with overlapping fingers (the index finger over the third finger and the fifth finger over the fourth) are associated with trisomy 18, Edward syndrome.

  • A 45-year-old woman, gravida 3, para 2, at 18 weeks' gestation comes to the physician for a prenatal visit. Ultrasonography at a previous visit when she was at 12 weeks' gestation showed a hypoplastic nasal bone. Pelvic examination shows a uterus consistent in size with an 18-week gestation. Maternal serum studies show low α-fetoprotein and free estriol concentrations and increased inhibin A and β-hCG concentrations. Physical examination of the infant after delivery is most likely to show which of the following findings?

  • Isoniazid (INH) needs mycobacterial catalase-peroxidase to get activated. So, if the bacteria reduce or mutate this enzyme, INH won’t work. INH normally blocks mycolic acid synthesis → destroys mycobacterial cell wall. But if activation fails → drug becomes useless. Others Ethambutol: ↑ Arabinosyl transferase Pyrazinamide: Mutated/lost pyrazinamidase Rifampin: Mutation in RNA polymerase Streptomycin: Altered 30S ribosomal protein

  • A 23-year-old man comes to the emergency department with fever, severe headache, and vomiting.  He has had fatigue, intermittent low-grade fever, and headaches for 2 weeks.  The patient is HIV positive.  He was incarcerated in a county jail for several months 3 years ago.  Temperature is 38.6 C (101.5 F).  Neck stiffness is present on physical examination.  CT scan reveals no intracranial lesions.  Cerebrospinal fluid analysis is notable for the following: Glucose24 mg/dL Protein180 mg/dL Leukocytes120/mm3 Cerebrospinal fluid cultures grow Mycobacterium tuberculosis with significantly decreased activity of intracellular catalase-peroxidase.   The isolates would most likely exhibit resistance to which of the following agents?
