Tip no. 285 ✨ توضیح کوییز Would rather برای بیان ترجیح استفاده میشود و بهمراتب قویتر از prefer است. این ساختار دو کاربرد اصلی دارد: 1. بیان ترجیح شخصی 2. بیان ترجیح دربارهی عمل دیگران 🟦 Structure A — ترجیح شخصی Subject + would rather + base verb Examples: - I’d rather start with a mild analgesic before prescribing anything stronger. - The team would rather monitor your vitals for a few hours before deciding on admission. - I’d rather not increase the dosage until we see how you respond to the current treatment. نکته مهم - فعل بعد از would rather همیشه باید base form باشد. - در حالت منفی: would rather not + base verb - استفاده از to کاملاً ممنوع است. 🟦 Structure B — ترجیح دربارهی دیگران Subject + would rather + subject + past simple / base verb Examples: - I’d rather you come / came in tomorrow for a follow‑up so we can reassess the swelling. - The doctor would rather you didn’t stop / not stop the medication without discussing it first. - I’d rather you avoid / avoided heavy exercise until the inflammation settles. نکته مهم زمان past simple در این ساختار معنی گذشته نمیدهد؛ فقط برای ایجاد فاصلهی دستوری و بیان ترجیح دربارهی دیگران استفاده میشود. 🟦 Structure C — Would rather + than برای مقایسه، بعد از would rather فقط از than استفاده میشود. (حرف اضافهی to برای ساختارهای prefer بهکار میرود.) Subject + would rather + base verb + than + base verb Examples: - I’d rather prescribe physiotherapy than start opioids at this stage. - She’d rather continue monitoring than undergo surgery right now.

Example 1: “You need to take the antibiotics as prescribed, otherwise the infection may worsen.” Example 2: "It's important to attend your follow-up appointment; otherwise, we won't be able to monitor your progress."
Tip no. 282 A review of common vocabularies 1. Linger IPA: /ˈlɪŋɡə/ Farsi Meaning: طول کشیدن، باقی ماندن، ماندگار شدن Example: Symptoms such as fatigue may linger for months after a viral infection. --- 2. Manslaughter IPA: /ˈmænˌslɔːtə/ Farsi Meaning: قتل غیرعمد، کشتن بدون قصد قبلی Example: The investigation examined whether the physician’s extreme negligence could amount to manslaughter under medical‑legal standards. --- 3. Obsolescence IPA: /ˌɒbsəˈlesəns/ Farsi Meaning: منسوخشدگی، از رده خارج شدن Example: The hospital replaced several diagnostic machines due to the obsolescence of their outdated software. --- 4. Cohort IPA: /ˈkəʊhɔːt/ Farsi Meaning: گروه، دسته، گروه مورد مطالعه Example: A cohort of 1,200 patients with hypertension was followed for five years to assess cardiovascular risk. --- 5. Panacea IPA: /ˌpænəˈsiːə/ Farsi Meaning: درمانِ همهچیز، راهحلِ جامع Example: Researchers caution that no single medication is a panacea for complex chronic diseases like diabetes.
Tip no. 281 سه شاخص مربوط به Information Giving در OET بهصورت کاملاً واضح و طبیعی در دیالوگ زیر نشان داده شده است. 🩺 Doctor–Patient Sample Dialogue Doctor: Before we go over your treatment plan, could you tell me what you already know about high blood pressure? (E1 – Establishing what the patient already knows) Patient: I know it’s something to do with the heart working too hard, but I’m not sure why it happens. Doctor: That’s a good starting point. You’re right, it’s when the heart has to pump against increased pressure in the arteries. (Pauses briefly) Sometimes this pressure builds up because of factors like stress, diet, or genetics. Does that make sense so far? (E2 – Pausing periodically and checking understanding) Patient: Yes, I think so. I didn’t realize stress could cause it. Doctor: Exactly. Stress can play a big role. How do you feel about managing stress in your daily routine? (E3 – Encouraging the patient to contribute reactions/feelings) Patient: I guess I could do better, I often skip relaxation time. Doctor: That’s perfectly understandable. We’ll talk about some simple ways to reduce stress later on. (Signalling Transition)
🩺 Sample WITHOUT discourse markers “Your symptoms are common in people with mild asthma. Your lungs are reacting strongly to cold air and exercise. They become irritated easily, which leads to the tightness you’ve been feeling. Using your inhaler before activity will help prevent those episodes. I’d also like you to keep a diary of when the symptoms occur. If they become more frequent, come back sooner. Most patients improve once they follow these steps. Continue your medication as prescribed and avoid sudden exposure to cold air for the next week.” --- 🩺 OET Advice Sample (WITH discourse markers) “To start with, I want you to know that your symptoms are quite common in people with mild asthma. Now, the thing is, your lungs are reacting more strongly than they should to cold air and exercise. In other words, they become irritated easily, which leads to the tightness you’ve been feeling. Basically, using your inhaler before activity will help prevent those episodes. At this stage, I’d also like you to keep a simple diary of when the symptoms occur. So, if you notice they’re becoming more frequent, please come back sooner. That said, most patients improve quickly once they follow these steps. To wrap up, continue your medication as prescribed and avoid sudden exposure to cold air for the next week.”

Tip no. 279 گزیده ای از British Medical Journal ■ مرور معانی کلمات مهم - Appalled — /əˈpɔːld/ وحشتزده، منزجر، شوکهشده - Leniency — /ˈliːniənsi/ رأفت، نرمگیری، تخفیف در مجازات - Non‑custodial — /ˌnɒn.kʌˈstəʊ.di.əl/ غیرحبسی، بدون زندان (مثل حکم خدمات اجتماعی) - Sentence — /ˈsentəns/ حکم قضایی، میزان مجازات - Convicted of — /kənˈvɪktɪd ɒv/ محکوم به… - Multiple counts — /ˈmʌltɪpəl kaʊnts/ چندین فقره اتهام - Rape — /reɪp/ تجاوز جنسی - Criminalise — /ˈkrɪmɪnəlaɪz/ جرمانگاری کردن - Reintegration — /ˌriːɪntɪˈɡreɪʃən/ بازپیوست، بازگشت فرد به جامعه - Serve justice — /sɜːv ˈdʒʌstɪs/ اجرای عدالت - Rightfully — /ˈraɪtfəli/ بهحق، عادلانه - Court of Appeal — /ˌkɔːt əv əˈpiːl/ دادگاه تجدیدنظر - Medical Tribunal Service — /ˈmedɪkəl traɪˈbjuːnəl ˈsɜːvɪs/ هیئت رسیدگی انضباطی پزشکی - Perpetrator — /ˈpɜːpɪtreɪtə/ مرتکب جرم، عامل جنایت - Sexual violence — /ˈsekʃuəl ˈvaɪələns/ خشونت جنسی - Injustice — /ɪnˈdʒʌstɪs/ بیعدالتی - Echoed — /ˈekəʊd/ بازتاب یافت، تکرار شد - Former consultant — /ˈfɔːmə kənˈsʌltənt/ مشاور سابق / پزشک متخصص سابق - Suspension — /səˈspenʃən/ تعلیق، محرومیت موقت از کار ■ The exerpt: We have been appalled recently at the leniency of the non-custodial sentences given to two teenage boys convicted of multiple counts of raping two teenage girls. The rapes happened in Fordingbridge, UK. Judge Nicholas Rowland wanted “to avoid criminalising these children unnecessarily and support their reintegration into society.” Our legal system failed to serve justice when the boys’ futures were valued more than justice for the girls who were raped. This case has now rightfully been referred to the Court of Appeal for review. The criminal justice system and the medical tribunal service are alike in valuing the lives of male perpetrators of sexual violence above the lives of women who have been raped.Injustices for survivors are echoed in medicine. Former consultant Aloaye Foy-Yamah was found to have raped a woman at his home but was given only a 12 month suspension by the Medical Tribunal Service (MPTS).

Tip no. 278 "Give your definite/confirmed diagnosis" VS. ✅For a preliminary diagnosis, these phrases show uncertainty, clinical reasoning: - “It seems like your symptoms might be related to a viral infection, but I’d like to confirm that.” - “It’s likely that this is a muscle strain rather than a fracture.” - “My initial impression is that you may be experiencing gastritis.” - “What I suspect is a mild allergic reaction, but we’ll check to be sure.” - “At this stage, it looks like a tension headache rather than something serious.” - “The most likely explanation is a viral infection, but we’ll confirm.” - “Given what you’ve told me, this sounds like a migraine.” - “Considering your symptoms, I suspect a viral infection.” - “Putting everything together, it appears to be a mild asthma flare‑up.” ✅How vocabulary signals a definite diagnosis: - “Based on the results of your blood test, you have iron‑deficiency anaemia.” - “Your symptoms are consistent with sinusitis.” - “The findings suggest that you have a urinary tract infection.” - “I confirm that this is a case of strep throat.” - “The tests confirm that your cholesterol levels are high.” - “The examination clearly indicates that the pain is due to gallstones.”

Tip no. 277 استفاده از اصطلاحات ساده و قابلفهم (Layman’s Terms) در بخش Speaking آزمون OET بسیار مهم است، زیرا مستقیماً بر نمرهٔ شما در معیارهای ارتباط بالینی، بهویژه ارائهٔ اطلاعات، مناسببودن زبان و ایجاد رابطه، تأثیر میگذارد. به بیان ساده: اگر بیمار نتواند حرف شما را بفهمد، نمره از دست میدهید. پس در حین ارائه اطلاعات به بیمار، نکات روی کارت را به ساده ترین بیان پارافریز کنید: مثال: ■Give diagnosis of ganglion cyst (benign fluid-filled sac, small tear in tendon membrane allowing contents to protrude, possibly caused by trauma to wrist, often self-limiting) معادل های ساده و قابل فهم کلمات کلیدی: ●Benign= this kind of lump is not dangerous ●Ganglion cyst= a small, harmless bubble filled with a jelly‑like fluid. (It usually forms near a joint, most often on the wrist.) ●Small tear in tendon membrane= tiny weak spot or small tear in the covering of a tendon or joint. ●Allowing contents to protrude= a bit of the normal fluid inside slowly pushes out ●Possibly caused by trauma to wrist= Sometimes this can happen after bumping or straining your wrist ● Often self-limiting= it goes away on its own without treatment.
Tip no.276 🗂️ OET Speaking Flow Checklist (Information-Gathering Phase) 1) Signal your intention (Start by explaining why you’re asking questions.) - “I’d like to ask you a few questions to understand your situation better.” - “I’ll ask you some questions so I can get a clearer picture of what’s going on.” - “Would it be okay if I asked you a few questions to learn more about your condition?” --- 2) Transition into questions (Use an open-ended question first, then narrow down.) - “To begin with, could you tell me what brought you in today?” - “What seems to be the problem?” - “Could you tell me a bit more about what’s been happening?” --- 3) Explore key areas (Move from general to specific questions.) 3.1) Symptoms / impact - “Can you describe how this is affecting your daily life?” - “What symptoms have you noticed?” 3.2) History taking - “I’d like to ask about your medical history, have you had anything similar before?” - “Have you experienced this problem in the past?” 3.3) Lifestyle / effect on routine - “Has this condition affected your sleep or appetite?” - “Has it changed your ability to work or carry out your usual activities?” 3.4) Severity - “On a scale of 1 to 10, how severe is the pain?” - “How bad would you say the symptoms are?” .... --- 4) Check if anything important has been missed - “Before we move on, is there anything else you’d like to share?” - “I don’t want to miss anything — is there another concern you’d like to mention?” - “Is there anything else you think I should know?” --- 5) Summarise and bridge to the next phase (Show you have listened and transition smoothly.) - “Thank you for sharing all that. Based on what you’ve told me, I’ll give you a brief summary and then discuss the next steps.” - “Let me summarise what I’ve understood so far, and then we can talk about what happens next.” - “Thanks, that’s very helpful. I’ll just summarise your concerns before moving on.” --- 🔊🔊🔊 Rapport-building During questioning, you should show empathy and make the patient comfortable. Examples: “I’m sorry to hear that.” “That sounds really difficult.” “Let’s see how I can help.” ....
