  • Explaining capnography Capnography uses infrared absorption spectroscopy to measure CO2 levels in exhaled air. Mainstream devices measure CO2 directly from the airway using a sensor at the endotracheal tube. Normal Capnographic Waveform: 1. The normal range for end-tidal CO2 (EtCO2) is between 35-45 mmHg. The waveform is divided into phases: 2. A-B phase: Respiratory baseline, where no CO2 is present. 3. B-C phase: Expiratory upstroke, as alveolar air reaches the sensor. 4. C-D phase: Expiratory alveolar plateau, representing CO2 from the alveoli. 5. D-E phase: Inspiratory down stroke. Common Waveform Abnormalities: 1. Inadequate CPR: Characterized by low CO2 levels, indicating the need for improved chest compressions. 2. Adequate CPR with ROSC: Shows a sudden increase in CO2, indicating return of spontaneous circulation. 3. Sudden Total Loss of CO2: Suggests issues like cardiac arrest, disconnected equipment, or esophageal intubation. 4. Sudden Decrease of Waveform: May indicate airway obstruction or disconnection. 5. Decreasing End Tidal CO2: Associated with hyperventilation, hypothermia, or decreased pulmonary perfusion. 6. Increasing End Tidal CO2: Linked to hypoventilation, increased metabolism, or conditions like CHF and DKA. 7. Obstruction: Caused by kinked airways, foreign bodies, or bronchospasm. 8. Inadequate Seal Around Endotracheal Tube: Results from leaks or improperly sized tubes. 9. Elevated Baseline-Increased CO2: Can be due to faulty equipment or poor mask seal. 10.
