ايش الفرق بين _Incarcerated hernia _Strangulated hernia طبعا في الحالتين الفتاق يكون irreducible ولكن الفرق في التروية الدموية... 🛑Incarceration: It is an irreducible hernia where the contents of the hernial sac are trapped outside the abdominal cavity and cannot be pushed back in, but without any compromise to their blood supply. يعني التروية الدموية مازالت سليمة ومافي اي موت للانسجة 🛑​Strangulation: It is an advanced and surgical emergency stage of an incarcerated hernia where the trapped tissue is compressed so tightly that its blood supply is cut off, leading to ischemia التروية الدموية انقطعت وفي علامات موت للانسجة وممكن يحصل gangrene

ايش الفرق بين لما نقول ال swelling is compressible ولما نقول swelling is reducible: 🛑​Compressible: _Disappear partially or completely on pressing the whole swelling & return to its normal size on releasing pressure يعني بمجرد ما ينتهي المؤثر اللي ضغط عليها ترجع تلقائيا... 🛑Reducible: _Decrease in size or disappear when compressed into certain direction & reappear only on cough يعني بمجرد ما ينتهي المؤثر ماترجع الا بمؤثر جديد مثل ال cough, straining

🛑Remember _appendicular pain aggrevated with movement _renal and ureteric pain reilieved by movement

ومتى نقدر نقول ان ال chest tube مش كافي وضروري نفتح وبالأصح Indications of thoracotomy - if there is a drainage of more than 2000ml of blood after insertion of tube. - if there is bleeding more than 200ml/hr for 3 Successive hours - if the chest x-ray reveals complete pacification of whole hemothorax or wide mediastinum>8cm - if chest trauma associated with other injuries as oesophagus or there is also cardiac tamponade
