Most common DRUG causing dependence?
Correct Answer - D Lithium carbonate is the current agent of choice, particularly during the manic phase. Because the onset of action is slow, concurrent use of antipsychotic agents such as chlorpromazine or haloperidol may be necessary to control mania. Concurrent use of tricyclic antidepressants may be necessary in the depressive phase. Monitoring of lithium levels is necessary because of the serious nature of the adverse effects (neurologic, renal, cardiac). Ref: Ropper A.H., Samuels M.A. (2009). Chapter 57. Depression and Bipolar Disease. In A.H. Ropper, M.A. Samuels (Eds), Adams and Victor's Principles of Neurology, 9e.

Ambivalence is the inability to decide for or against, due to co-existence of two opposing impulses for the same thing at the same time in the same person. It is an important feature of schizophrenia. It is also seen with OCD, and other borderline conditions. Abulia - Inability to make a decision generally. Unlike ambivalence, there are no opposing impulses present. Anhedonia - Loss of pleasure in previously pleasurable activities. Alexithymia - Inability to recognize and describe feelings.
3. A patient says she loves her child and wants to hug him but also hates him and wants to hurt him. She is unable to decide what to express. This is known as:
Obsessive-Compulsive Disorder (OCD) is a neurotic disorder and not a psychotic disorder Patients with OCD recognize the irrational nature of the behaviour (insight is present) but cannot stop and hence suffer from significant distress and functional impairment. In psychotic illnesses. there is no insight. Note: OCD cannot be strictly considered as a neurotia disease because 1. All the OCD patients do not have good insight, some patients can have poor or absent insight. 2. Few OCD patients can present with psychotic phenomena such as delusions and hallucinations. OCD was grouped under neurotic,stress-related and somatoform disorders in ICD 10 classification. However, DSM 5 and ICD 11 na longer have a group called neurotic disorders. OCD and Related disorders are classified as a separate entity in both
