    🩺 ◈Bad Obstetric History (BOH)◈ ➤ Bad Obstetric History (BOH) refers to a situation where a woman has had unfavorable outcomes in her previous pregnancies, which may increase the risk of complications in the current or future pregnancies. 📌  Bad Obstetric History (BOH) : ➤ A history of one or more previous pregnancies that resulted in adverse outcomes such as repeated pregnancy loss, stillbirth, neonatal death, or congenital abnormalities. ⚠️ Key Components of BOH: ➽ A pregnant woman is considered to have BOH if she has experienced one or more of the following: ◈ 1. Recurrent Pregnancy Loss (RPL): △ Loss of two or more consecutive pregnancies before viability (usually before 20–28 weeks depending on context) ▩ Causes may include: ➽ Genetic abnormalities ➽ Hormonal imbalance ➽ Uterine abnormalities ➽ Infections ◈ 2. Stillbirth ➤ Fetal death after 28 weeks of gestation (WHO definition). ➠ Cause ➽ Placental insufficiency ➽ Hypertensive disorders ➽ Maternal infections ◈ 3. Early Neonatal Death: ➽ Death of a newborn within the first 7 days of life. ➽ Often associated with: ➤Prematurity ➤ Birth asphyxia ➤ Severe infections ◈ 4. Congenital Anomalies Previous baby born with structural or functional defects. Such as:- ➽ Neural tube defects ➽ Congenital heart disease ◈ 5. Preterm Birth: ➽ Delivery before 37 completed weeks of gestation. ➽ Repeated preterm births increase BOH risk classification ◈ 6. Intrauterine Growth Restriction (IUGR) ➽ Baby is smaller than expected for gestational age Indicates placental or maternal problems. 7. Abruptio Placenta / Placenta Previa History ➽ History of placental complications in previous pregnancy. ◈ 8. Unexplained Fetal Loss Previous pregnancy loss with no identifiable cause. 🧠 Clinical Importance of BOH ➽ Women with BOH are classified as high-risk pregnancies because: Increased chance of recurrence. Higher maternal anxiety and psychological stress Need for specialized antenatal care and monitoring. 🔍 Common Causes of BOH 🧬 Genetic Factors ➤ Chromosomal abnormalities in parents or fetus 🦠 Infections ➤ TORCH infections (e.g., Toxoplasmosis, Rubella) 🧪 Endocrine Disorders ➤ Diabetes mellitus Thyroid dysfunction 🩸 Immunological Causes ➤ Antiphospholipid syndrome 🏥 Anatomical Factors ➤ Uterine malformations ➤ Cervical incompetence 🩺 Management Approach 1. Preconception Care Counseling and risk assessment, and Folic acid supplementation 2. Antenatal Care (ANC) ➤ Early booking ➤ Frequent follow-ups ➤ Ultrasound monitoring 3. Targeted Investigations ➤ Genetic testing ➤ Infection screening ➤ Hormonal evaluation 4.     ◈ FOLIC ACID (Vitamin B9) IN PREGNANCY◈ 🧬 WHAT IS FOLIC ACID? ➽ Folic acid is a water-soluble B vitamin. ➽(Vitamin B9) essential for: ➨ DNA formation ➨ Cell division ➨ Rapid tissue growth (especially in early pregnancy) 👉 It is most critical in the first 28 days of pregnancy, often before a woman even knows she is pregnant. 🌸 ADVANTAGES OF FOLIC ACID 👩‍⚕️ For the Mother ➳ Prevents megaloblastic anemia ➳ Supports healthy placenta formation ➳ Reduces risk of pregnancy complications. 👶 For the Baby ➠ Ensures proper brain and spinal cord development ➠ Supports normal fetal growth ➠ Reduces congenital abnormalities ⏰ TIMING OF FOLIC ACID 🟢 BEFORE PREGNANCY (Preconception) ➤ Start at least 1–3 months before conception Critical because neural tube forms very early. 👉 This is the MOST IMPORTANT PERIOD 🔵 DURING PREGNANCY ➽ 1st Trimester (0–12 weeks) Highest importance Neural tube closes by day 28. ➽2nd & 3rd Trimester Continue to support: ➳ Blood formation ➳ Fetal growth 💊 DOSAGE RECOMMENDATION ✅ Normal Pregnancy ➠ 400 micrograms (0.4 mg) daily ⚠️ Bad Obstetric History ➠ (BOH) / High-Risk Women Includes: ➽ Previous baby with neural tube defect ➽ Recurrent miscarriage ➽ Family history of congenital anomalies Diabetes, epilepsy, obesity 👉 Recommended dose: 5 mg daily (HIGH DOSE). 🛡️ DEFECTS PREVENTED BY FOLIC ACID 🧠 Neural Tube Defects (NTDs) 1. Spina Bifida Spine does not close properly May cause paralysis 2. Anencephaly Brain fails to develop Usually fatal 3. Encephalocele Brain tissue protrudes outside skull ❤️ Other Conditions Reduced ◈ Congenital heart defects Cleft lip & palate ◈ Some urinary tract defects. 🔴 FOLIC ACID IN BOH (SPECIAL FOCUS) Why higher dose? ➤ Women with BOH have: △ Higher recurrence risk of birth defects △ Possible underlying nutritional deficiency 💡 Benefits in BOH ▩ Reduces recurrence of NTDs by up to 70% ▩ Improves fetal survival in early pregnancy ▩ Supports proper implantation and placental development. 🍃 NATURAL SOURCES OF FOLATE ◈ Dark green vegetables (spinach, kale) 🥬 ◈ Lentils and beans ◈ Citrus fruits 🍊 ◈ Liver (in moderation) 👉 BUT diet alone is NOT enough → supplementation is required.
